Data speaks but we do not listen

In a recent analysis of over 990 safety recommendations, it became clear that one of the main issues is not a lack of knowledge, but a lack of informed action. Safety management cannot rely solely on what we "believe" works. It must be based on what we know works, and data is the bridge to that knowledge.
